Preparing for Your First Visit
When you arrive at our office for the first time, we'll need to acquire certain information about your child's health and dental insurance coverage before we begin treatment. Please arrive 15 minutes early to complete the insurance and medical history forms.
For your convenience, you can also download the forms here and complete them at home.

Most initial exams include a complete cleaning and last approximately one hour. You'll be provided with oral hygiene instructions that will help you care for your teeth. We'll also perform an evaluation and look for potential dental problems. If we spot something, we'll propose a course of treatment. To schedule your appointment, please call us at 256-883-5050.
Financial Information
We strive to provide you with affordable, excellent treatment that fits your budget. We offer a variety of payment options for your convenience.
We'll explain your options thoroughly. If you have any questions, please let us know.
Insurance
We're committed to helping you maximize your insurance benefits. Due to the complexities of insurance benefits, we can only estimate your coverage in good faith, but we cannot guarantee it. We will bill your insurance company for services and give them 45 days to render payment.
- Your estimated portion must be paid at the time of service.
- After 60 days, you are responsible for the entire balance, paid in full.
If you have any questions, please call us at 256-883-5050.
Appointments
When you schedule an appointment with us, we set aside that time specifically for you. If you are unable to keep your scheduled appointment, please give us 24 hours' notice to allow us to use that time for another patient. A fee may be charged if you do not give us notice. We understand that unforeseen events may cause you to miss an appointment.
Emergency Information
We provide emergency dental care to our patients of record. If your child has a dental emergency, please call our office at 256-883-5050 as soon as possible. Please do not come directly to the office — calling us allows us to create a time to see you. Even if you have a regular appointment scheduled, please call ahead.
Broken Teeth
If your child has one or more broken teeth, rinse the area with warm water. Put a cold compress over your child's face around the area of the injury. Recover any broken tooth fragments and call us immediately.
Bleeding After a Baby Tooth Falls Out
Fold a piece of gauze, place it tightly over the bleeding area, and bite down on it for 15 minutes. If the bleeding continues, please call our office.
Knocked-Out Permanent Tooth
Recover the tooth as soon as possible, making sure to hold it by the crown (the tooth end) and not by the root. Rinse it, but do not clean or handle it more than necessary. Reinsert the tooth in the socket and hold it in place with a clean piece of gauze or cloth. If you cannot reinsert the tooth, carry it in a cup of milk, saline or saliva. In either case, contact us immediately. Time is essential in treating dental trauma like this.
